We envision a Silicon Valley where all have a fair chance at leading a healthy life and a person’s income, race, immigration status, and zip code are not barriers to health.
We know the answers to achieving health equity will come from those most proximate to the issues and have lived experience. We work in partnership with the community and the nonprofit public and private sectors.
Types of grants we offer
Health Partnership Grants
Supporting organizations and projects for medically-related purposes that align with The Health Trust's mission and focus areas of food and nutrition, chronic disease management, housing support, and other approaches to building health equity.
Submit a Concept Form first. Selected applicants are invited to submit a full proposal.Health Equity Fund
Supporting timely, high-need projects that may fall outside our main focus areas but clearly address health disparities and advance health equity. Requests must demonstrate a clear connection to health, supported by relevant research, data, and community need.

How to ApplyCommunity Grants
These grants support organizations, projects, and events that align with our mission. Grants provide a direct benefit to people disproportionately impacted by the social determinants of health.
Apply for up to $5,000. Submit requests at least 60 days before your event or project start date.

Our Focus Areas
At The Health Trust we believe that no person should ever go hungry. We are committed to partnering with organizations throughout Santa Clara and Northern San Benito counties to invest in food system infrastructure to meet residents’ food and nutritional needs. Supporting organizations that remove barriers to health food access, and supporting policies, practices and systems that strengthen healthy food policies and the local food ecosystem.
Access to good healthcare that serves an individual and meets them where they are is key to healthy communities. Chronic Disease management is critical to improving health outcomes. The Health Trust supports enhancing infrastructure and capacity of clinical providers and community-based organizations especially focused on prediabetic, type 2 diabetes, and HIV/AIDS. Supporting policies and programs that focus on health outcomes, increasing guidelines-based STI/HIV screenings and rapid connection to care.
Many living in Santa Clara and Northern San Benito counties face difficult decisions of paying rent or supporting critical basic health needs. The Health Trust strives to reduce the pressure of making those difficult choices. Our goal is to support affordable housing organizations through infrastructure, capacity-building, system-level change investment to amplify health and wellness services for residents, and to invest in policy advocacy efforts that result in stronger health-related services that support residents in affordable and supportive housing.
